Promoted fields not showing up in SharePoint library

The setup: we have a SharePoint 2007 farm and we want to publish two forms to a library.  It was set up that it would use content type for the drop down, so the forms are templates.  For one form, everything works accordingly.  User click on the New DropDown, select the form.  Fill out the form and submit.  The fields are submitted and all the promoted properties show up accordingly, i.e. employee's first name, last name, etc..

The second form same set up, but when users submit the form.  He does appear in the library, but all the promoted fields are blank.  No error message, no hiccup.  I created a new content type and did the same process, same thing.  Took the form to a 2007 test farm to recreate and things are working fine, no issue.  So I know the form is correct.  I know the process is correct. 

I am wondering if I hit a threshold, but the numbers are pretty low, 8 promoted properties.  There are 11 items already in the drop down.  Not sure what else I need to do.  Form works fine internally, it is that no fields are promotoed, so employee's first name, last name, etc. are completely blank. 

Any help would be appreciated.
